Name/Address/Birthdate Changes
In order for an educator in South Dakota to change his or her name, address,
and/or birthdate on his or her South Dakota Teacher’s Certificate, he or she must
complete the "Request for Name/Address/Birthdate
Change" form.
All sections of the form must be completed as per the information
requested. Completed forms must be sent to the Office of Accreditation & Teacher Quality,
Attn: Teacher Certification, 700 Governor’s Drive,
Pierre, SD 57501-2291. If the educator desires to have a new teacher’s
certificate issued, he or she must send a $20 money order or personal check
along with the form.
