your email address. Retype your email address in the box that
says Retype Email address. If the computer gives you an error
message that the email addresses don’t match, check to make sure
they are typed exactly the same and that there are no extra
spaces at the beginning or end of either email address.

like your name to appear on your nametag

how you would like your name to appear on your certificate of
completion

Click on one of the Local Agency
program categories - Child & Adult Care Food Program (CACFP) or
National School Lunch Program (NSLP) or Other. If you choose
other, type in the name of the program.

Check male or female for the
gender of participant.

Check whether participant wants a
single or double occupancy room or if no room is needed.

If Double Room
and participant has roommate preference, list
Roommate Name and Local Agency.

If Double Room
and no roommate preference, a roommate will be
assigned to the participant.

Meals must be purchased in order
to attend Child Nutrition Institute as it is important to be at meals
to hear announcements that relate to the participants such as
attendance details, upcoming events, classroom changes, etc. as well
as it is a great place to relax and network with other food service
participants.

Participants may bring
guests to the
Thursday evening banquet. A graduation ceremony will be held at the
banquet for those participants who complete track 4.

If the participant is bringing guests to
the Banquet, type the number of guests in the box next to Guests.

Banquet
cost per guest is $13.00 and may be paid with registration fees
or paid separately at registration on June 22.

Click on the box next to Special
Accommodations if any medical need requires accommodations. Type
what accommodations are needed in the box provided.

Click
the box next to indicate requests for alternative formats and/or
interpreters if you have a need for any of the listed items.

If you
have other access requests, please describe them in the text
box. Examples include refrigerator access for medicine, or not
being able to walk very far due to disability. If you would like
your ServSafe test in a language other than English, please
write that in the box.

Click the box next to the Track
the participant is registering for. Only one Track may be selected.

If registering for
Track 1, participant must also purchase a ServSafe book with the
ServSafe exam in it. The price of the book will automatically
be charged to the participant when Track 1 is selected unless
the participant does not need a book or has already taken the
exam in which case the box may be selected that says “I already
have a book” and the price of the book will not be included in
the registration costs.

If participant would like to
attend the free Math Review Session on Sunday, June 22, select the
box next to Math review

If the participant would like to be
an Attendance Clerk, check the box next to attendance clerk.

- Please make sure that all costs
are filled in correctly for how you intended to register. If
the selections are not what you intended, please hit the back arrow
and go back to the page with that item on it and select a different
option. For example, if you marked that you would like to order
Banquet tickets, but you changed your mind or don’t want them
included with your registration paperwork, you could go back and
uncheck the box and they will be removed from the last page.
- Note:
Online registration must be completed before early bird registration
date and payment must be postmarked by early bird deadline in
order to receive the early bird rate.
