What are Transition Services?
Transition services are a coordinated set of activities for a student with
a disability promoting the movement from school to post-school activities.
Transition Services Liaison Project
The Transition Services Liaison Project (TSLP), is a
state initiated project offered through the two State Vocational
Rehabilitation (VR) agencies –Division of Rehabilitation Services (DRS)
and Service to the Blind and Visually Impaired (SBVI) and the State
special education agency – Special Education Programs (SEP). The
managing agency grantee is the division of rehabilitation services which
contracts with Black Hills Special Services Cooperative for project
management. The Transition Services Liaison Project can provide technical
assistance and training to students, families, LEA's, and adult service
agencies.

Regional Transition Liaisons
The state of South Dakota has three regionally based staff to provide
support and technical assistance to individuals, families, LEA's, and
agencies seeking information on transition planning. Activities provided
by the Regional Transition Liaisons can include: training to students,
families, schools, and adult service agencies on transition and
development of transition plans; identifying and obtaining instructional
materials and assistance with implementation of self determination
curriculum; assistance with identification of local resources; development
of interagency councils to address regional transition service needs; and
individual support/assistance.

Transition Tacklebox
The Transition Tacklebox was disseminated across South Dakota, Fall of
2001. These
guides are intended to serve as a resource to educators, students and
their families, agencies, service providers and potential employers,
giving them the information they need to provide successful secondary
transition planning and services. The tacklebox includes the
following: Educators In Industry, Promising Practices by Meade County,
LIFE Manual (Transition Manual), Self Advocacy Guide, Community Based
Instruction, Project Skills information, Catch The Wave, Parent Manual,
Tips for Rehab Counselors, and the Assistive Technology Guide.

These guides were disseminated at the following
locations:
SDUAP, SD Parent Connection,
ONE Stop Centers, BIA Schools, 121 Projects, educational
cooperatives, colleges (with teacher resource centers), independent living
centers, tribal colleges and private schools.
What is Project Skills?
Many students with significant disabilities don’t get an opportunity to
gain paid employment experience while in high school. Although willing,
most employers cannot afford the supports these students frequently
require on their first job. This is an important learning, maturing,
socializing experience. The Division of Rehabilitation Services (DRS)
maintains a partnership with the local school districts to address this need
and funds a program entitled "Project Skills". During the first
year of this program over 150 students with disabilities received paid
employment experience. Project Skills significantly impacted their
readiness for adult life. Forms and documents used in Project Skills are:

Participant Manual ; Work Experience Agreement ; Assurance of Match Report
; Monthly Service Report, and Project Skills Sample Contract Sample.
What is Youth Leadership Forum?
The Youth Leadership Forum for Students with Disabilities (YLF) is a
unique career leadership training program for high school juniors and
seniors with disabilities to serve as delegates from their communities.
Students with disabilities will cultivate leadership, citizenship, and
social skills as a result of participating in this five-day event. The YLF
is implemented at the state level by the Governor’s Committee on
Employment of People with Disabilities, state and local partners.
What is the YLF all about?
YLF is an educational and motivational forum with an intense schedule.
Small "working groups" explore personal leadership and career
plans. Social, artistic, athletic, and recreational activities abound, as
these are part of leading a well-rounded life. Guest speakers address such
topics as disability rights laws and innovations in technology. A farewell
dance will allow a time to celebrate accomplishments with new friends.
The key to the YLF is leadership by
example. Adults with disabilities who have traveled the same path these
young people are facing serve as group leaders and staff. Many other
volunteers from various backgrounds assist to make the
program a success.
