Title I Annual LEA Evaluation Report

Download the  2000-2001 Title I Evaluation Report Form and Instructions here.

The Evaluation Report is due on or before June 15 ,2001.  Contact Gary Skoglund at 605-773-5229 if you have any questions.

Title I evaluation data is collected at the end of each school year for all grade levels served by the program. Information such as poverty levels of schools, delivery systems, eligible and participating student numbers, staff information etc. are just some of the categories. Assessment information is collected at Grades 4, 8 and 11. Criterion referenced performance level scores are gathered from The Stanford Achievement Test Series, Ninth Edition. The levels are Below Basic, Basic, Proficient and Advanced. This data along with local evidence of progress are used to define adequate yearly progress. Schools failing to show a 5% increase in students moving from Below Basic to Basic or Basic to Proficient for two consecutive years will be placed in school improvement. Targeted assistance schools may choose to report all students in a class receiving Title I assistance or only those served. Schoolwide programs must report all students in the grades served.

In addition to the end of school year evaluation a Title I Performance Report is sent to every school in early September. This report is a record keeping tool for schools and teachers to use. The last two summary pages of the Performance Report must be attached to the spring evaluation when it is sent in.

Summer school evaluations must be completed at the end of the summer program. Schools must request a summer school evaluation from the Title I office if they are conducting a program.
