Long-term sub report due May 15

State law requires the Department of Education to collect long-term substitute data from all accredited schools annually. The deadline to complete your districtís 2010-11 Long Term Substitute submission is May 15, 2011. The submission can be accomplished by using the Personnel Record Form, or PRF.

A long-term substitute is an individual employed as a substitute teacher for 10 or more consecutive school days for the same teacher. You should not report substitutes for administrators, school service specialists or other types of school personnel.

To complete and submit your district report, click here. You will need your PRF log-in and password.

If you do not know your district log-in and password, please speak with your local PRF contact or contact Jennifer Rattling Leaf or Carla Leingang at the department.
