GED® verification forms updated for 2014-15 school year

The Department of Labor and Regulation verifies students’ education status before students under age 18 can take the GED® test. The verification forms have been updated.

• A home school parent/guardian would complete page one of this document to authorize a student to complete the GED® test. DLR will confirm with the Department of Education that a current public school exemption certificate has been submitted by the school district.

• A school district would complete page two of the document, authorizing a 16- or 17-year-old student who has not been excused from public school attendance for alternative instruction (home school) to take the GED® test. By signing the form, a school administrator verifies that the student will not graduate with his or her cohort because of credit deficiency.

• Other institutions, like the Department of Corrections or Job Corps, or a court service officer, would also complete page two of the document.
