Transition services are a coordinated set of activities for a child with a disability, based on the individual child s needs, taking into account the child s strengths, preferences, and interests, and designed to be within a results-oriented process.
Transition services focus on improving the academic and functional achievement of a child with a disability to facilitate the child s movement from school to post-school activities.

- Transition Resources for Students with Disabilities - Vocational Rehabilitation in the South Dakota Department of Human Services works with South Dakota school districts and other organizations within the state to help students with disabilities plan for their future. A variety of services are available to help match skills, abilities, and interests with a compatible career path.

Assistance with daily living
- CHOICES waiver Community, Hope, Opportunity, Independence, Careers, Empowerment, Success (CHOICES) is a program that provides services to children and adults with intellectual and developmental disabilities
- Family Support 360 - Family Support 360 is not a single service, but rather a flexible constellation of services and supports which are customized to meet the varied and changing needs of each participant and family
- Shared Living (Department of Human Services) Shared living is an arrangement of services provided in the private home of a participant or a family, in which care is provided to a participant. Shared living is meant to provide a more person-centered approach to supports that are offered to a participant where the participant and the shared living provider share a life.
- Guardianship (Department of Human Services) - A state-sponsored program with financial assistance available to assist adults with disabilities in establishing a guardianship or conservatorship
